Already have a primary care doctor you're happy with? You don't need to switch. Our stand-alone program gives you physician-supervised weight management while you keep your current doctor for everything else. The program includes one visit per month, in the clinic or by telehealth, plus phone and text access for questions about your weight management. Your current doctor remains your go-to for other health concerns, and with your permission we're glad to keep them informed of your plan.

To check your progress, adjust your plan, and monitor how you're tolerating treatment (stand-alone program: one visit per month, in the clinic or by telehealth).
Direct access to your physician
Reach Dr. Jebelev by phone or text between visits with questions about your plan or side effects (for stand-alone patients, this covers weight management topics).
Medication Options
Not everyone needs medication, and medication is prescribed only when it's medically appropriate for you. When it is, Dr. Jebelev will talk through the options, the expected costs, and what to expect — and help you choose what fits your health and your budget.
FDA-Approved Medications
Depending on your health history and coverage, options may include:
Weekly injections:
Wegovy® (semaglutide) or Zepbound® (tirzepatide)
Daily tablets:
Wegovy® tablets (semaglutide) or Foundayo™ (orforglipron)
For patients with type 2 diabetes:
Ozempic® (semaglutide) or Mounjaro® (tirzepatide)
Prescriptions are sent to the pharmacy of your choice. Your cost depends on your insurance coverage, manufacturer programs, or eligibility for the Medicare GLP-1 Bridge (see below).
Compounded Medications
For some patients, a compounded medication prepared by a licensed compounding pharmacy may be appropriate. Compounded semaglutide and tirzepatide injections are among the options we can discuss, including cost, at your visit.
Compounded medications are not FDA-approved. The FDA does not review compounded drugs for safety, effectiveness, or quality. Compounded medications are prescribed only when medically appropriate for the individual patient.
For Medicare Patients: The Medicare GLP-1 Bridge
For years, Medicare drug plans generally haven't covered medications prescribed for weight management. That changed for many people on July 1, 2026, when Medicare launched the Medicare GLP-1 Bridge, a temporary federal program that runs through December 31, 2027.
If you qualify, you pay $50 per month for certain GLP-1 medications at participating pharmacies, including:
- ·Wegovy® (injection and tablets)
- ·Zepbound® KwikPen®
- ·Foundayo™
Who May Qualify
You must be enrolled in Medicare drug coverage (a stand-alone Part D plan or a Medicare Advantage plan with drug coverage), be 18 or older, and meet Medicare's clinical criteria at the time you started GLP-1 treatment:
- ·A BMI of 35 or higher, or
- ·A BMI of 30 or higher with heart failure with preserved ejection fraction, uncontrolled high blood pressure, or chronic kidney disease stage 3a or higher, or
- ·A BMI of 27 or higher with prediabetes, a prior heart attack or stroke, or symptomatic peripheral artery disease
If you have type 2 diabetes, obstructive sleep apnea, or MASH (a type of fatty liver disease), GLP-1 coverage generally goes through your regular Part D plan instead of the Bridge.
How We Help
The Bridge requires a prescription and a prior authorization submitted by your doctor. As part of the program, Dr. Jebelev evaluates whether a covered medication is right for you, confirms whether you're likely to meet the criteria, prescribes, completes the prior authorization, and follows your progress.
You can fill a Bridge prescription at participating pharmacies. Amazon Pharmacy, for example, joined the program in August 2026 and offers home delivery. The $50 copay is set by Medicare and is the same at participating pharmacies.
Good to Know
- ·The $50 copay does not count toward your Part D deductible or out-of-pocket maximum, and Extra Help (Low-Income Subsidy) does not apply to it.
- ·Approval is determined by Medicare, not by Clear Access MD.
- ·Clear Access MD does not bill Medicare. Our $99/month program fee is paid directly under a private agreement, and Medicare does not reimburse it. Your Medicare coverage and your relationship with your current doctor stay the same.

We build your plan together, handle prescriptions and paperwork, and follow up regularly to fine-tune what's working.
